Tear Film & Surface Support

Lacrisert®

hydroxypropyl cellulose ophthalmic insert 5 mg

Clinical Snapshot

Lacrisert is a sustained-release hydroxypropyl cellulose insert placed in the inferior cul-de-sac that provides prolonged lubrication throughout the day. It is an option for patients with moderate-to-severe dry eye who require continuous surface support beyond what drop-based therapy can provide.

Mechanism of Action

The hydroxypropyl cellulose insert slowly dissolves in the tear film, increasing tear film viscosity and providing sustained lubrication. This reduces the need for frequent drop instillation and provides more consistent surface coverage.

Common Clinical Applications

  • Moderate-to-severe dry eye disease
  • Patients requiring sustained lubrication beyond drop-based therapy
  • Keratoconjunctivitis sicca

Typical Dosing Principles

One insert in each eye once daily, placed in the inferior conjunctival sac. Some patients may benefit from twice-daily use.

Dosing information is provided for educational purposes. Always consult current prescribing information for specific dosing protocols.

Important Precautions

  • Transient blurred vision following insertion is common and should be discussed proactively.
  • Insertion technique requires patient education and practice.
  • Discomfort or foreign body sensation may occur, particularly initially.

Clinical Pearls

  • Useful for patients who cannot comply with frequent drop instillation schedules.
  • Blurred vision after insertion typically clears as the insert begins to dissolve.
  • A useful option for patients with severe aqueous deficiency requiring sustained surface support.
  • Insertion technique is a teachable skill — in-office demonstration improves compliance.
