A 67-year-old man with a 20-year history of poorly controlled hypertension and type 2 diabetes presents with sudden, painless, complete loss of vision in his right eye that began 45 minutes ago. He denies pain, flashes, or floaters. On examination: VA OD counting fingers at 1 foot, OS 20/25. Pupils: right RAPD present. Fundus OD: diffuse retinal whitening with a cherry-red spot at the fovea. IOP 16 mmHg OU.
