Treated water is not sterile water. This Masterclass examines the paradox at the center of aquatic eye care: the same chemical processes that make recreational and municipal water "safe" for human use create conditions that can compromise the ocular surface, select for resistant microbial populations, and trigger inflammatory cascades that present in clinic long after the exposure event. Drawing on environmental chemistry, microbiology, and ocular surface physiology, this course reframes aquatic keratoconjunctivitis as a systems-level problem — not simply an infection to treat, but an exposure pattern to understand.
Clinicians regularly encounter patients with red, irritated eyes following aquatic exposure — and frequently attribute the presentation to simple chemical irritation or a self-limiting viral conjunctivitis. This course challenges that reflex. Understanding how chlorination byproducts interact with the tear film, how biofilm-adapted organisms evade standard disinfection, and how repeated aquatic exposure can destabilize the ocular surface ecosystem gives clinicians a more accurate diagnostic framework and a more defensible management approach.

Upon successful completion of this educational experience, the clinician will be able to:
Describe the chemical mechanisms by which common water disinfectants — including chlorine, chloramine, and bromine — interact with the ocular surface and tear film.
Identify the microbial species most commonly associated with aquatic keratoconjunctivitis and explain why standard disinfection protocols do not reliably eliminate them.
Explain the concept of disinfection byproduct formation and its relevance to ocular surface inflammation.
Apply a clinical framework for evaluating patients with aquatic exposure history, including differentiation of chemical, microbial, and mixed etiologies.
Recommend evidence-informed management strategies for aquatic keratoconjunctivitis, including patient counseling on exposure risk reduction.
